Any advice on how to make a career change?

I am a social worker with a Master's degree and five years managerial experience in a health care setting. I want to change careers into something more creative such as writing, marketing, the arts, etc. Any advice on making the change and making my skills transferable to a different career?Any advice on how to make a career change?
I believe many of your skills will transfer... especially your communication skills. I'd create a list of your achievements over the years and that see what skills were demonstrated as a result. I'm sure you'll see how some can easily transfer... and you'll want to list those abilities on your resume. Most employers use electronic surveying of key words to get through resumes, so make sure you use those key words when you can.
